Admissions

Once the parent has submitted all necessary paperwork, an interview is scheduled with the administration. The administration
will send the parent a letter granting or denying admission. If a student is denied, all personal records will be returned
to the parent.

These items are required for enrollment:
- Copy of last report card, achievement test scores, and discipline records (1st-12th)
- Certificate of Immunization on state form #3231 (all grades)
- Ear, Eye, and Dental on state form #3300 (K5-12th)
- Birth certificate
- Two chickenpox vaccinations or proof of immunity on state form #3189 (entering K5 and 6th)
- Second MMR vaccination or proof of immunity (entering 6th)
- Pastoral reference letter (7th-12th)
- Payment of non-refundable processing fee
- Payment of non-refundable tuition insurance fee
- Completed forms in enrollment packet
- Interview with administration
